Option 1:
You may apply for a West Virginia license by choosing the appropriate application type from the list below. Applications are completed online through Certemy, our licensing management software provider, and include a number of steps to be completed and submitted for verification and approval. Application must be completed in its entirety. Failure to do so may result in delay and/or denial of license.
Initial licensure fees for a WV license are $75 for OT and $50 for OTA.

Option 2:
IF your home state is an ACTIVE member of the OT Compact, and you have a current license in good standing in your home state, you may be eligible to purchase a privilege to practice in West Virginia. To see a list of states actively participating in the Compact, click here.
Your privilege to practice in WV will expire with your home state license and must be re-purchased through the OT Compact to continue to practice in WV. Privileges to practice DO NOT renew automatically upon renewal of your home state license.
Fees to purchase a privilege to practice include a $75 fee paid to the OT Compact Commission plus a WV state fee of $50, for a total of $125 each time purchased.
**OT PRACTITIONERS MAY CONTINUE TO HOLD A LICENSE TO PRACTICE IN ANY STATE UNDER EXISTING LICENSURE LAWS, WITHOUT JOINING THE COMPACT.**
